Prostate Cancer
Each case is unique and treatment decisions depend upon the stage and grade of the disease. Each patient and his family are counseled thoroughly and introduced to all of the appropriate options. Treatment can range from observation with active surveillance to the various forms of radiation therapy, including intensity modulated radiation therapy (IMRT), radiation seed placement or high dose rate brachytherapy (HDR). For patients who elect to undergo surgery, Dr. Figlesthaler performs the daVinci® robotic prostatectomy. Having performed over 1,200 of these procedures, Dr. Figlesthaler is one of the most experienced robotic surgeons in the region. Emphasis during the procedure is on curing the cancer by removing the diseased prostate, while sparing the nerves and urinary sphincters so that bladder control and sexual function successfully recover.
Prostate Enlargement (BPH)
As men age, it becomes more common to feel the need to urinate more often and more urgently, and eventually have difficulty in completely emptying the bladder. These and other symptoms can be caused by an enlarged prostate or BPH. After a thorough evaluation, the condition can often be managed with dietary changes, natural supplements and mild medications, but minimally invasive surgery such as Rezūm Water Vapor Therapy can offer more permanent solutions and eliminate the need for medications.
Kidney & Bladder Cancer
Cancer of the urinary tract is common and often requires surgical treatment as well as lifetime surveillance. All of our surgeries are performed either laparoscopically or using robotic minimally-invasive techniques. This allows for sparing of the healthy tissues by removing the tumors and preventing damage to the healthy portions of the bladder or kidneys.
Kidney Stones
Kidney stones can be caused by numerous physiological defects and are often painful. We use an endoscopic approach with laser and ultrasonic lithotripsy for ureteral stones and large kidney stones as well completely noninvasive shock wave lithotripsy for small and medium-sized kidney stones. Once we clear all of the stones, we perform special urine and blood tests to determine what specific changes can be made in diet or medications to prevent future attacks.
Erectile Dysfunction (ED)
This is one of the most common conditions that effect men of all ages and for many different reasons. There is often both a physiological as well as psychological component to this problem. Color duplex doppler scans can be used to determine if there is proper blood flow to the penis, and treatment may be as simple as oral medications or hormone replacement, but in severe cases outpatient surgery may be an option, including endovascular surgery or placement of a penile prosthesis.
Hormone Replacement Therapy
Low energy levels, weight gain, especially in the abdominal area and waist as well as loss of libido and sexual desire can all be symptoms of a testosterone deficiency. Total testosterone levels do not always tell the complete story, and a thorough evaluation is needed before beginning therapy. Once appropriate hormone levels are restored, the results can be life changing.
No-scalpel Vasectomy & Reversals
The no scalpel vasectomy is one of the simplest and painless treatments for a lifetime of birth control for couples that have completed their families. The procedure takes approximately 15 minutes in the office and normal activities can be resumed within a few days. For those who change their minds or decide to start a new family, we perform the vasectomy reversal using microsurgical techniques. We confirm the presence viable sperm cells at the time of surgery.
Vaginal Rejuvenation & Repair
Bladder control problems affect more than 60% of women over the age of 40 years. Often these problems result from pregnancy and childbirth, which cause enormous stress to the pelvic anatomy. Various defects such as cystocele, rectocele and vaginal vault prolapse can all be associated defects. After a thorough evaluation, all of these conditions can be corrected in a single outpatient vaginal surgery, thus restoring the pelvic and vaginal anatomy to its previous state.
Bladder Control Problems
The involuntary loss of urine can be caused by many different anatomical and physiological problems, including childbirth, prolapse, prostate problems, diabetes, strokes, etc. Often there may be a combination of issues. Computerized urodynamic studies as well as digital flexible cystoscopy offer additional information in more complex cases. Treatments may include dietary modifications, biofeedback, medications or in some cases minimally invasive surgery, such as the sling procedure.
Hematuria (Blood in the Urine)
Blood in the urine can be visible (gross) or invisible (microscopic). It can be due to many causes both benign as well as malignant. We perform a painless, but thorough evaluation of the entire urinary tract, including a CT scan, special urine studies as well as digital flexible cystoscopy to determine the cause of this condition and then offer both an explanation and a solution.

"No Doubt I Strongly Recommend Him"
I have been going to several Doctors specializing in Urology over the past 4 years. All other Doctors said that my
enlarged prostate is normal for a man my age and that the only thing they could do is watch the PSA level.

In my case he performed first several tests at his office to determine there was no cancer cells in the Prostate.
He was then able to do a new procedure, Plasma Vaporization a noninvasive technique that gently vaporizes
the enlarged Prostate tissue and leaves behind a smooth surface. Now frequent urination is a thing of the
past with very few side effects and with the Prostate still fully functional.

"My wife and five children mean everything to me. Thoughts of them raced through my mind when I first heard the terrifying diagnosis... I had prostate cancer. It was because of them that I decided to take a leap of faith and have my prostate removed, Just one week later. I couldn't imagine not being able to do the things we loved to do together, like boating and traveling. It was the best decision I've ever made.
It all started when I went in for a routine checkup with my primary care physician. He noticed that my prostate-specific antigen (PSA) levels were elevated, indicating prostate cancer. "I was diagnosed with prostate cancer at age 56. I had no outward symptoms, just a slightly elevated PSA. After receiving shocking biopsy results, I was convinced the prostate needed to be removed. An appointment was scheduled with a surgeon.
I began to research in earnest to gain as much information as possible. My research showed the da Vinci robotic surgery success rate to be as good as traditional open surgery, great surgeons were having great results with both, but the robotic procedure was much less invasive. Since da Vinci was very new to my community at the time, I set out on an intensive research mission to find a great surgeon. Fortunately, I found Dr. Figlesthaler, a physician that embraces technology and innovation. I located his practice through many trusted sources and was very impressed with the background and experience of the staff. I initially set an appointment for a second opinion. I knew from my very first call that I was dealing with a very compassionate staff. After meeting with Dr. Figlesthaler and his staff, the second opinion turned quickly to a full confidence that this is the practice that I needed. All options were discussed, surgery was scheduled quickly. I felt completely at ease with my choice of physician.
The toughest part of the surgery was the anxiety spent waiting for the procedure. The surgery day was a blur. The postoperative night was very comfortable, thanks to a great nursing staff. I was discharged within twenty-four hours. I was groggy, but had no pain. I improved daily after surgery. I did have a minor set back when the catheter was removed after two days. It was replaced and removed after seven days, with no adverse effects. My improvement permitted me to go back to my office part-time the same week of surgery. I was at work full-time within a couple of weeks. The healing and function improved every day. The best news came when my post surgery PSA was undetectable, just as predicted. That was one of the best days of my life!
I must say the staff was extremely comforting and professional at every turn. We did have occasion to call the office very early one morning concerning the catheter and late, one Saturday night, with questions. Our calls were returned within five minutes. All concerns were promptly, professionally and compassionately addressed.
I cannot say enough about Dr. Figlesthaler and the staff of this practice. They are the BEST.
